Section 2: Scholarship Selections

Please select the scholarships you will be applying for. You can only apply for a maximum of 3. Note: All scholarships on the website are not available every year and are based on funding levels.

Section 3: Personal Statement (written or by video)

In 1000 words or less (or 2-4 minutes of video), answer each of the following questions: (1) why and how your academic pursuits will help you positively impact the broader community; (2) why you want to go into your chosen field (i.e., education, technology, etc.); (3) describe your community service; (4) what are your academic and professional goals; and (5) is there anything else we should know relative to this scholarship.

Section 4: Letters of Recommendation

To support your application, please submit two (2) letters of recommendation by individuals who can attest to your character, accomplishments and personal/professional pursuits. Each letter must be on letterhead, signed and should provide contact information for the recommender including name, address, phone number and email. (NOTE: Recommendation letters signed by current MEDF Trustees will not be reviewed/accepted.)

· One letter from a teacher, counselor, principal, professor or dean/provost
· One letter from an organization evidencing community service signed by the President or other lead administrator

· Media Release form (electronic signature is permitted)
· High School and/or College/University Transcript (Unofficial is acceptable)
· Two letters of recommendation

Section 6: Special Requirements For Specific Funds

Doris B. Corsey Arnold Child Care Fund	Must submit notarized letter from childcare provider evidencing childcare expenses.
Kay F. Hopkins Scholarship Fund	Essay must address why the applicant decided to pursue education and the impact the applicant hopes to make on future scholars.
James and Patricia Solomon Memorial Fund	Must submit a letter evidencing that applicant is a member of Plymouth Congregational Church.
Michelle Watson Memorial Scholarship	Essay must describe the challenges the applicant has overcome or difficulties the applicant is working to address.
